Inter's league form is equally clean but their attacking numbers look more volatile: a 5-3 win over Udinese and a 3-2 against Napoli suggest they have been scoring freely, though those scorelines also tell their own story defensively.

Roma are missing Pellegrini (thigh) and Bah (knee), which hurts more than the names might suggest. Gasperini's system asks a lot of its wing backs and midfielders in terms of athleticism and recovery and both are exactly the profile he needs most. Inter have their own injury headaches: Çalhanoğlu's adductor strain is the most significant and without him the midfield loses its organiser and the side's best set piece taker. Stones and Spence are also out, thinning the back three and the wing back options and leaving Barella and the remaining central defenders carrying extra responsibility.

Roma line up in a 3-4-2-1 or 3-4-1-2 depending on the phase, building through high pressure. Malen leads the line and has scored six times in four league games, which is a number worth pausing on, while Dybala operates in the half space behind the striker. Cristante sits deep to allow the wing backs to push higher, though without Pellegrini, Roma's attack loses its third runner and without Bah the flanks need covering on the transition.

Chivu's Inter counter with a 3-5-2 that leans on Lautaro Martinez and Marcus Thuram, a partnership that has already scored eight times in four league games. Barella carries more creative burden than usual with Çalhanoğlu gone and the wing backs become more important in both attack and cover. The Real Madrid defeat showed how quickly Inter can move from back to front when they have space to run into and any overcommitment from Roma's press leaves exactly the kind of space that Thuram and Martinez are built to exploit.

Malen against Inter's central defenders is the match up to watch given his pace in behind is a persistent problem for any back three and Dybala versus Barella in the central zones will likely decide who controls the tempo. Cristante also has work to do staying between the lines when Martinez and Thuram combine. Roma have the setup to press Inter into mistakes but Inter have the quality to punish hesitation when it comes.

Roma's last home Serie A win over Inter came in 2016 and Inter won 5-2 at the Olimpico as recently as April 2026, which makes that a difficult record to look past. Without Çalhanoğlu, Inter's set piece delivery loses some accuracy, which mildly helps Roma, though Roma are also missing aerial presence at both ends with Pellegrini and Bah out. Roma's +11 goal difference against Inter's lower return is the clearest indicator of which side has been more dominant within games, not just in results.

A 1-1 draw is the likeliest outcome. Roma's press and Malen's finishing should create enough to get on the scoresheet at home and Dybala has the movement to pull Barella out of position or force Inter's back three to step up. But Thuram and Martinez have already scored eight between them in four games and don't need sustained pressure to score and both squads are short handed enough that neither can realistically chase the game, leaving a draw as the result that keeps both unbeaten runs intact.
